Board Credit Forms
This form is to be used by individuals or groups seeking to gain approval by the Staff Development Committee to conduct a course for Board Credit. This form should be sent to Justin Lopez at firstname.lastname@example.org. Board Credit Courses are approved throughout the year, as appropriate by the Staff Development Committee. Each course must have a minimum enrollment of 15 for classes to proceed or "go". Courses where there is no cost for the instructor may seek approval to proceed with less than 15 participants. Courses with close to 15 participants may seek approval to proceed from Dr. Laura Taylor at 217-351-3702.
This form is to be used by individuals once they have completed a course that has been approved for Board Credit in order to receive earned Board Credit. The form should be submitted to the instructor of the course who will verify participation by signing the form and sending it to Justin Lopez at the Mellon Building to be approved by Dr. Taylor. Dr. Taylor will confirm the information and will forward the form to Human Resources so the participant can receive Board Credit.
This form is to be used by individuals planning on taking a workshop or course that has not been approved for Board Credit. The form must be submitted to Justin Lopez at email@example.com for PRIOR approval by the District Professional Development Committee.
