The following health examinations are required for Champaign Unit #4 students:

Physical Examination – Required for PreK, Kindergarten, Sixth and Ninth Grade students, as well as any student entering from out-of-state.

Dental Examination – Required for Kindergarten, Second and Sixth Grade students.

Eye Examination (performed by a licensed optometrist) – Required for Kindergarten and any student entering an Illinois school for the first time from out-of-state or from being home-schooled.
